Registered Nurse Residency Program - Careers At Nascentia Health (2024)

This program can support candidates in Onondaga and Oneida counties and their respective surrounding counties.

Registered Nurse Resident (RNR) is a novice nurse that participates in a nurse residency cohort. The RNR is responsible for learning how to provide direct patient care, coordination of overall home care services and documentation of care for agency clients in accordance with Quality Standards and under the guidance of a Clinical Supervisor. After the successful completion of 6 months in the residency program, you will be required to work one weekend a month. At the end of the 12-month residency program the novice nurse will be a competent nurse in home care and advance to the position of Registered Nurse Case Manager.

Performance Responsibilities and Standards:

  1. Participates in the nurse residency program and the educational opportunities offered through classroom, online, simulation and in home experience.
    • Completes all required assignments, online learning modules and learning activities as assigned.
  2. Learns how to ensure quality and safe delivery of home health services.
    • Learns how to complete a head-to-toe assessment while assessing for environmental concerns and social determinants of health that can affect the patient’s overall wellbeing in a home environment.
    • Learn how to develop a plan of care based on client assessment and OASIS evaluations.
    • Learn how to develop a plan of care using interdisciplinary approach.
    • Ensure that all documentation meets quality standards:
    • Completes all OASIS evaluations.
    • Documentation reflects nursing care and interactions.
    • Documentation is clear and concise.
    • Documentation is completed in timely manner
    • Understand and follow nurse code of ethics and standards of practice.
    • Learn to coordinate and collaborate with internal and external resources to meet patient needs.
    • Develops an in depth understanding of community-based resources.
    • Maintain established productivity expectations.
    • Communicate client care issues to interdisciplinary team.
  3. Learns how to coordinate care among fellow RN’s and LPN’s.
    • Learns how to provide guidance and education in client care to LPN’s.
    • Work in conjunction with Clinical Supervisor and Clinical Coordinator to efficiently manage patients using a team approach while appropriately delegating patients to LPNs on the care team.
  4. Learns how to supervise paraprofessional staff relevant to patient care.
    1. Supervise home health aides in performing scope of practice appropriate tasks related to patient care and per care plan.
    2. Communicate appropriate information to home health aide department.
  5. Comply with Nascentia Health policies and procedures and quality initiatives.
    1. Maintain confidentiality.
    2. Demonstrate knowledge and use of infection control practices.
    3. Demonstrate knowledge of federal, state, and local government rules and regulations.
    4. Demonstrate knowledge and compliance with agency policies and procedures.
    5. Adhere to quality improvement standards.
    6. Keep personnel requirements up-to-date, i.e., health requirements, licensure requirements, and other Nascentia Health requirements.
    7. Demonstrate skills competency through periodic evaluations.
  6. Participate in personal growth and development.
    1. Participate in educational in-services and meetings that are offered to clinical staff.
    2. Maintain and update clinical knowledge on an ongoing basis.
  7. Present a professional image to internal and external customers in compliance with Nascentia Health’s dress code.
  8. Perform all other duties as assigned.

Job Qualifications

  1. Current New York State registered professional nurse license or may hold a Graduate Nurse License.
  2. Must attain RN licensure within 90 days of hire.
  3. Degree from an accredited school of nursing or college.
  4. Current and valid driver’s license and auto insurance, and a vehicle which can be used for work.
  5. Self-directed and organized with the ability to work independently.
  6. Basic computer knowledge with proficiency in Microsoft Office or other Word processing, spreadsheet, and database management.
  7. Good oral and written communication skills.
  8. Demonstrates motivation to learn a new role and is an active learner.

Physical Requirements

  • Ability to sit or stand at a desk 90% of the day
  • Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs with a max of 150 lbs; lifting/carrying on average 25 lbs with a max of 40 lbs; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
  • Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to use telephone and communicate over the phone
  • Frequently uses a laptop computer to enter orders and obtain product information
  • Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids
